One of the most common criticisms of acupuncture is that any benefits people experience are simply the result of placebo. Spend enough time reading comment sections, and you’ll encounter the same argument, “it’s just placebo”.

Yet, despite the criticism, acupuncture continues to be studied in universities, hospitals, cancer centers, and research institutions around the world. Researchers have spent decades investigating when it helps, why it helps, and how to study it properly. And the last question turns out to be more complicated than many people realize.

In a recent discussion on the YouTube channel PAINWeek, pain specialist Dr. Heidi Reetz explores this very issue. Her argument was not that acupuncture avoided scientific scrutiny, but that acupuncture presents a unique challenge for researchers attempting to study it using methods originally designed for pharmaceutical treatments.

Understanding those challenges may help explain why the debate surrounding acupuncture persists.

A Research Model Built for Pills

Modern medicine research was built around a relatively simple model. One group receives a treatment, another receives placebo. Ideally, neither group knows which they received.

For medications, this process is relatively straightforward. Patients receive either a pill containing the medication or a pill that looks identical but contains no active ingredient. However, acupuncture presents a unique challenge.

Most people know whether a needle has been placed in their body, therefore, creating a convincing placebo becomes much more difficult than handing someone a sugar pill.

When the Placebo May Not Be a Placebo

As Dr. Reetz explains, researchers have developed a variety of approaches to address this challenge. Some studies place needles in locations that are not traditionally used in acupuncture. Others use shallow needle insertion, brief needle placement, or devices designed to mimic the sensation of acupuncture without fully penetrating the skin.

The difficulty is that many of these control interventions may not be completely inactive.

Some researchers argue that even light stimulation of the skin, pressure applied to specific areas, or brief needle insertion may produce physiological responses of their own. If true, researchers may sometimes be comparing one intervention against another intervention, rather than comparing treatment against nothing at all.

This does not make acupuncture impossible to study, it just makes the method more complex.

More Than the Needle

The conversation becomes even more complicated when researchers consider everything that occurs during acupuncture treatment. People spend time resting, they receive focused attention from a practitioner, and they often enter an environment designed to encourage relaxation.

Whether these factors should be considered part of the treatment or separated from it remains an ongoing topic of discussion. But adds another layer of complexity, how do researchers isolate the effect of the needle itself from the larger treatment experience? A question without a single answer.

The Treatment Series Debate

Dr. Reetz also highlights another challenge that receives less attention. Acupuncture is rarely delivered as a single treatment. Rather, it is typically provided as a series of treatments over time.

Researchers continue to debate how many treatments are necessary to fairly evaluate acupuncture’s effects and whether shorter study designs accurately reflect how acupuncture is used in real-world practice.

In other words, understanding whether a treatment works may depend in part on understanding how that treatment is actually delivered.

Research and its Continuing Evolution

Perhaps the most important takeaway is that researchers have not ignored these challenges, they have spent decades attempting to address them.

In recent years, scientists have developed sophisticated methods, including specialized non-penetrating needles and even double-blind acupuncture needles, designed to address longstanding concerns about placebo controls and blinding. Which we saw in the first ever double-blind acupuncture trial conducted at the University of Illinois Chicago this year.

What was once considered impossible is now becoming achievable. The result is a growing body of research that continues to become more rigorous and better equipped to answer difficult questions.

Why It Matters

Scientific skepticism plays an important role in medicine. Questions and criticism both help strengthen research and improve our understanding of what works and why.

The conversation surrounding acupuncture is often framed as a debate between believers and skeptics. But as Dr. Reetz points out, the reality is much more nuanced. The question is not simply whether acupuncture has been studied, the question is how researchers can best study a treatment that does not neatly fit into conventional research models.

And perhaps that’s one reason acupuncture remains an active area of scientific investigation today. Researchers are not only studying how it works, they are continuously refining how to answer that question.

What This Means for You

The ongoing debate about acupuncture may tell us as much about the challenges of studying complex treatments as it does about acupuncture itself.

After all, scientific models are designed to help us understand the world, but they are not the world itself. When something doesn’t fit neatly into an existing framework, it is often dismissed before it is fully understood.

So perhaps the question isn’t whether acupuncture fits inside the box. Perhaps it’s whether the box is large enough to hold every meaningful avenue of healing.
